In FairConsent, you can add children, adult wards, and witnesses to a consent record to ensure accuracy, transparency, and compliance. Children and Wards can be added under a legal guardian to group related individuals within a single consent record. Witnesses help confirm that consent was properly obtained and are typically staff members, local partners, or any trusted adult.
When recording consent for children, adult wards, or witnesses, the guardian’s consent must be recorded first.

1. Recording a Child or Adult Ward
After You Collect The Guardian Consent, Add a Child or Adult Ward
- Once the guardian has confirmed their consent, tap "Add child", or "Add adult ward".
Enter Information
For each child or ward, complete the following details:
- Name
- Age (in years or months)
- Gender
- Relationship to the legal guardian
- Notes (optional, e.g., project-specific details)
- Tap Continue to capture a photo of the child or ward, then save the information.
Add Multiple Children or Wards
Tap "Add child" or "Add adult ward" again to include additional individuals under the same guardian
Repeat the process for each person
2. Recording a Witness
Add a Witness
- After completing the consent process, tap "Add witness".
Enter Witness Information
Provide the following details:
- Name (New or Existing)
- Gender
- Phone number
- Email address
- Notes (optional)
- Tap Confirm to proceed
N.B.: Selecting an existing witness from the "Name" field, will auto-fill the other fields

Confirm Witness
Witnesses can confirm their role by:- Taking a photo with a thumbs-up gesture, similar to consent confirmation
- This step adds an additional layer of verification and accountability.
N.B.: Always ensure that guardian consent is recorded before adding children or wards, and include a witness whenever required for compliance or project standards.
